d. 持有有效的年度执业证书；且

f. 持有公共卫生高级文凭/证书/后基础资格 - 健康状况：
- 申请者必须获得招生机构认可的注册医师出具的健康证明
- 患有HIV/AIDS、乙肝或丙肝、任何精神健康障碍、癫痫及其他慢性疾病者不得入学
- 不得有任何影响护理操作能力的身体缺陷或感官障碍申请者需通过林肯大学学院英语入学测试；或
i. 具备同等水平的英语(Bahasa Inggeris)能力
